Thermal Expression
What is Thermal Expression
Radiofrequency (RF) and devices such as the UMAY Rest are in-office treatments that focus on one of the main causes of dry eye: blocked or underactive meibomian glands.
These glands are found along your eyelids and make the oily layer of your tears. This oil helps slow down how quickly your tears evaporate. If the glands get clogged or the oil thickens, which is called meibomian gland dysfunction, your tears become less stable. This can cause dry eye symptoms like burning, grittiness, blurry vision, and irritation.

How it Works
Radiofrequency (RF) and devices such as the UMAY Rest are in-office treatments that focus on one of the main causes of dry eye: blocked or underactive meibomian glands.
These glands are found along your eyelids and make the oily layer of your tears. This oil helps slow down how quickly your tears evaporate. If the glands get clogged or the oil thickens, which is called meibomian gland dysfunction, your tears become less stable. This can cause dry eye symptoms like burning, grittiness, blurry vision, and irritation.
The main goal of RF or thermal devices is to gently heat the glands to a specific temperature (about 107 to 108°F). At this warmth:
-
Thickened, toothpaste-like oils inside the glands begin to melt and loosen.
-
Blockages soften, making it easier for the glands to clear.
-
Blood flow in the surrounding tissue may improve (especially with RF), supporting healing and gland function.
Radiofrequency has an added benefit beyond just heat. It delivers gentle energy into the tissue, which can:
-
Promote collagen remodeling in the eyelid skin.
-
Improve lid tightness and positioning.
-
Enhance overall gland expression efficiency.
Once the glands are adequately warmed, the second step—manual expression—is crucial. Your provider applies controlled, targeted pressure along the eyelids to physically release the melted, trapped oils. Think of it like clearing clogged pores, but in a very precise, medically guided way. This step:
-
Removes stagnant, unhealthy oil.
-
Allows fresh, healthier oil to flow.
-
“Resets” the gland function.
After treatment, many patients experience:
-
More stable tear film (tears don’t evaporate as quickly)
-
Improved eye lubrication and comfort
-
Reduced need for artificial tears
-
Clearer, more consistent vision
It’s worth noting that results can be cumulative. Some patients benefit from a series of treatments followed by maintenance sessions, depending on how advanced their gland dysfunction is.
This treatment doesn’t just relieve symptoms temporarily—it works to rehabilitate the glands so your eyes can produce a healthier, more stable tear film on their own.
